Unlock H.E.A.V.E.N. mode:
Unlock Heaven-A, -B and -C - Beat Gryphus (stage 8 boss) and go through the door behind you when you re-enter Primordium (level 0) after the resulting cutscene.
Unlock the Highest Floor:
Beat all 99 levels of H.E.A.V.E.N.
H.E.A.V.E.N. (A, B and C):
Defeat the Gryphus boss in stage 8. Next, go back to Level zero (Primordium) after
the intermission and go to the door behind you.
